Hi,
I recently created a form with a single field (Email) to collect the email addresses of contacts who were interested in a new edition of our product. A number of people have filled out the form over the last few weeks. See below:

However, this form does not show up on their contact record in Eloqua. See the examples below, these are the first three people from the above screenshot - schaerli@, adnan@, and brandonhall@:

Additionally, and much more troubling, is that while they are shown as submitters of that form, they do not receive credit on their lead score for having filled out this form. They each do have a lead score on their record, but this particular form is configured to give them 100% of the engagement value in the lead score algorithm. They receive no credit.
Can someone help explain the circumstances that would cause these submissions to not show on the contact record, how to fix that, and resolve my question about lead scoring in regards to these records? Please let me know. Thanks!
