If you build yourself a simple segment I think you will have a easy time with this. For the first criteria, add the name of your form and then just throw in the other criteria you are searching for. If you have a few different counts you are trying to put together, you add each one to this same segment as a different filter criteria and Eloqua will provide you with a count for each on the far-right.
Note: in order for this to work, your form must be posting all of the data you are searching for to either the contact table or a CDO.
