I have a solution that will give you the name of the landing page but not the URL. You need the Eloqua Analyzer license to do this work.
- To do so you would copy this report: Form Submission Data
- Go to the edit view of that report
- Choose "All Objects" from the bottom left
- Search for "Landing Page"
- Add the "Landing Page" metric to the report
